REQUIRED EXPERIENCE Position: RN – Nurse Case Manager. Minimum 1 year of experience in case management, social work, and discharge planning – Required. Cerner EHR proficiency – Mandatory.

Previous rural healthcare experience is strongly preferred. Must be comfortable coordinating patient care in a resource-limited healthcare environment. SCHEDULE

Days. Hours: 8:00 AM–4:30 PM.

5 × 8-hour shifts per week.
