Licensure
✓Active Registered Nurse (RN) license in New York State
Certifications
✓2–3 years of clinical experience in a Certified Home Health Agency (CHHA), Lombardi program, MLTC, or MAP program
✓Additional Requirement: PRI and Screen certification must be obtained within the first three months of employment
Education
✓Bachelor's degree required
BSN preferred
