REQUIRED EXPERIENCE Position: RN – Acute Care Case Manager. Minimum 2 years of experience required. Must have Acute Care Case Management experience in California – HARD STOP.
Kaiser experience is excluded from the qualifying California acute case management experience. Outpatient Case Management, Home Health Case Management, and Health Plan Case Management experience will not satisfy the requirement. Must have experience working in a hospital-based acute care environment.
SCHEDULE
Days, 8:00 AM–4:30 PM.
Monday–Friday. Shift pattern: 5 × 8-hour shifts. REQUIRED CERTIFICATIONS Note: BLS, ACM, and CCM LICENSE Active California RN License – Required.
