Shift: Days | Monday–Friday, 8:00 AM–4:30 PM📋

5×8s | 40 hours/week⏳ Contract: 13 weeks💰 Weekly Gross: $2,025.60/week Position DetailsSupport patients and families throughout the hospital discharge processAssess patient and caregiver readiness for dischargeRound on patients and communicate needs or concerns to the RNReinforce discharge education and ensure patients/families have necessary resourcesIdentify barriers to discharge and collaborate with the care team to resolve themHelp support safe and efficient patient flow and transitions of careServe as a liaison between patients, families, and the RNEach LVN will support a Med Tele unitUnits have approximately 24 beds, with caseloads generally ranging from 20–24 patientsFloating is required based on staffing needs.

Candidates must be comfortable floating as needed.
