Qualifications
✓Minimum 2 years of acute hospital Case Management experience
✓Recent acute care experience within the last year required
✓Epic charting experience required
✓Experience in care coordination and discharge planning
✓Experience with utilization review, admission criteria, and discharge planning
✓Knowledge of HMOs, IPAs, advance directives, POLST, and protective services processes
✓Every other weekend required
✓Floating based on department needs
✓Local candidates accepted
✓First-time travelers may be considered with clinical approval
