Santa Rosa Center for Rehabilitation and Healing: Address: 5386 Broad St, Milton, FL 32570 Sandy Ridge Center for Rehabilitation and Healing: Address: 5360 Glover Ln, Milton, FL 32570 Step Up Rehab is the in-house therapy provider at Santa Rosa Center for Rehabilitation and Healing and Sandy Ridge Center for Rehabilitation and Healing. Work inside the dedicated therapy gyms within these skilled nursing facilities, helping residents regain function and independence through skilled, compassionate care.

FULL-TIME POSITION: This position is exclusively for professionals licensed in the state of Florida as a COTA. All services are provided in a Skilled Nursing Facility (SNF). Provide skilled COTA-related therapy services to residents in a Skilled Nursing Facility while collaborating with the interdisciplinary team to improve safety, function, and quality of life.

* Graduate of an accredited COTA program * Active Florida COTA license (or license-eligible) required * SNF or post-acute experience preferred * Net Health knowledge preferred Please see the link below to the new Care Provider Background Screening Clearinghouse Education and Awareness website, implemented under House Bill 531 (2025)
