Must have RN active license (Any U. S. State or Territory) Must have AHA BLS Must have minimum of 2 Years or more experience in discharge planning and Med-Surg experience.

Must have RN experience in a clinical, medical/surgical, acute and/or critical care setting.

Performs case management and coordinates the discharge planning care process of age-appropriate in-patients with chronic diseases and special health care needs within Multi-Service Unit, Intensive Care Unit, OB Units and Adolescent Care Unit by maintaining a tracking system for referred patients for admission with other healthcare facilities by performing chart reviews for requested services for return to facility or discharge home.

8-Hour Shift Monday - Friday (8:00 am to 5:00pm).
