We are seeking a compassionate Behavioral Health Licensed Practical Nurse to join our supportive Worcester-area team.
In this role, you will provide personalized nursing care in patients’ homes under the direction of a Registered Nurse or physician. You will build meaningful relationships while helping patients manage their conditions, medications, and daily care needs.

Meaningful, one-on-one patient careGreater autonomy and scheduling flexibilitySupportive and collaborative clinical teamComprehensive onboarding and mentorshipCompetitive compensation (pay is based on experience and education with a range between $33 to $38/hr)Opportunities for professional growth and advancementMedical, dental, and vision insurance401(k) with employer matchPaid time off and paid holidaysTuition reimbursement and continuing education opportunitiesFamily and pet bereavement leavePet insuranceWhat You’ll DoAs a Behavioral Health LPN, you will:Review the patient’s Plan of Care before each visit and provide care according to RN delegation, physician orders, assessment findings, and established clinical standardsDeliver skilled nursing services requiring specialized nursing knowledge under the direction of a Registered Nurse or physicianIncorporate established patient goals into each nursing visit and document the care and progress accuratelyMonitor each patient’s physical, behavioral, emotional, and environmental conditionPromptly communicate significant findings, safety concerns, or changes in condition to the RN Case Manager, physician, Clinical Supervisor, and other members of the care teamReport unsafe conditions or concerns within the patient’s home environment to the appropriate clinical personnelSupport medication management by reviewing and documenting medication regimens and updating medication profiles according to established proceduresEducate patients and caregivers on appropriate self-care techniques, medications, symptom management, and safety practicesHelp patients and caregivers recognize warning signs and understand when to contact the clinical teamImplement patient-safety interventions using available family, caregiver, and community resourcesRecommend updates to the Plan of Care when new needs or concerns are identifiedCollaborate with the RN Case Manager for clinical guidance, support, and instructionsTake appropriate action to help prevent avoidable hospitalizations and emergency room visitsCommunicate professionally and compassionately with patients, families, caregivers, physicians, and agency personnelTreat patients, caregivers, and their personal property with dignity, respect, and careMaintain accurate, timely, and complete clinical documentationProtect patient and employee privacy and confidentiality in accordance with HIPAA requirementsFollow Elara Caring’s mission, policies, procedures, and quality-of-care standardsPerform additional duties and special projects as assignedWhat You’ll NeedHigh school diploma or GEDGraduate of an accredited practical nursing programCurrent, unrestricted Licensed Practical Nurse license in MassachusettsAt least one year of nursing experience in an acute-care or equivalent clinical settingBehavioral health or home health experience preferredCurrent CPR certification through the American Heart Association or American Red CrossStrong communication, organization, and clinical documentation skillsCompassionate, dependable, and dedicated approach to patient careAbility to work independently while collaborating with an interdisciplinary care teamReliable transportation, a valid driver’s license, and current automobile insuranceWillingness and ability to travel throughout the assigned Worcester-area territorySit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
