Responsibilities include:Performing comprehensive nursing assessments in patients' homes. Developing, implementing, and updating individualized plans of care. Providing skilled nursing care, treatments, and patient education.
Collaborating with physicians, therapists, and interdisciplinary team members to coordinate care. Monitoring patient progress and communicating changes in condition. Educating patients and caregivers to promote confidence and independence.
Maintaining timely and accurate clinical documentation. Delivering care in accordance with regulatory and organizational standards.
