Responsibilities include:Delivering care in accordance with physician orders, delegated nursing responsibilities, and the established Plan of CareMonitoring patient conditions and reporting significant changes to the RN Case Manager and healthcare teamAssisting patients and caregivers with education and self-care techniquesImplementing patient safety plans and utilizing available community resourcesCollaborating with the interdisciplinary care team to ensure exceptional patient outcomesDocumenting clinical findings, nursing interventions, and medication updates accurately and timelyIdentifying opportunities to improve patient outcomes and reduce avoidable hospitalizations and emergency room visitsProviding compassionate, professional support to patients, families, physicians, and colleaguesQualificationsRequired:Graduate of an accredited Certificate, Diploma, or Associate Degree Nursing programCurrent Massachusetts Licensed Practical Nurse (LPN) licenseMinimum of one (1) year of clinical nursing experienceValid driver's license, reliable transportation, and current auto insuranceAbility and willingness to travel throughout the assigned territoryPhysical Requirements- Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
