Provide supervision and direction for field staff and act as liaison between field and office staffMonitor clinicians via HCHB reportsUpdate Clinical Record on an ongoing basis as relates to communications, orders, re-hospitalizations, conflicts, etc. Evaluate and address customer concerns, including documentation and resolutionCommunicate with sales personnel regarding customer issues that arise and may affect referral source relationshipsResponsible for communication with clinicians and physicians when adult protective services referral requiredEvaluate client episode to determine appropriateness of discharges versus recertification and initiation of orders as appropriate.
Review and manage visit time exception requestsAttend all required interdisciplinary and staff meetings. All other duties as assigned by Clinical DirectorHome Health Clinical Field Staff Supervisor (RN)
Must hold, in good standing, a license as a Registered Nurse, issued by the state in which you work. A minimum of two years working as a Registered Nurse in a clinical care setting, hospice or home health environment preferred. One-year nursing supervisor experience preferred.
Must have strong communication and organizational skills and high standard of ethics and compliance. Ability to document within an electronic medical record. Must possess an Associate’s Degree from an accredited School of Nursing or applicable Degree for TherapyDue to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
