RN Clinical Manager – Home Health Full-Time | Braintree, MAAre you an experienced Home Health RN who loves developing people, improving patient care, and keeping a clinical team moving forward? As our Home Health Clinical Supervisor, you’ll guide and support field clinicians, coordinate high-quality patient care, and help your team deliver the outcomes patients deserve—all while staying connected to the mission that brought you into nursing.

Your responsibilities will include:Overseeing patient care, clinical staffing, referrals, and assignmentsEnsuring timely starts of care and seamless care coordinationReviewing plans of care, physician orders, and clinical documentationConducting OASIS reviews to support accuracy, quality, and complianceMonitoring patient needs and ensuring care plans remain appropriateCoaching clinicians and providing ongoing education and feedbackSupporting recruiting, interviewing, onboarding, and staff developmentPartnering with physicians, patients, families, providers, and vendorsMonitoring key clinical and operational performance metricsProviding field support when neededWhat You BringCurrent, unrestricted Massachusetts RN licenseAssociate degree in nursingAt least two years of RN experience in home health or hospiceSupervisory or clinical leadership experience preferredStrong OASIS, care coordination, and clinical documentation knowledgeA natural ability to coach, communicate, and bring out the best in othersStrong organization, attention to detail, and clinical judgmentAbility to learn systems such as Homecare Homebase and WorkdayDependable transportation, a valid driver’s license, and current auto insuranceWillingness to travel within the branch’s coverage areaAbility to sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
