🌲 Travel Physical Therapist (PT) – Home Health 📍 Cascade Locks, OR Ready for your next Pacific Northwest adventure? Ardor Health Solutions is seeking an experienced Travel Physical Therapist for a rewarding Home Health assignment in Cascade Locks, Oregon! This is a great opportunity for a PT who enjoys independent patient care while surrounded by the incredible scenery of the Columbia River Gorge.
Perform comprehensive physical therapy evaluations in patients' homes. Develop individualized treatment plans based on physician orders and patient goals. Help patients improve mobility, strength, balance, safety, and functional independence.
Provide education on home exercise programs, fall prevention, and safe mobility. Communicate patient progress and needs with physicians, caregivers, and the interdisciplinary team. Complete accurate and timely clinical documentation.

Requirements Active Oregon Physical Therapist license or Compact PT License. Home Health experience preferred. OASIS experience preferred.
HCHB experience preferred. Current BLS certification. Valid driver's license and reliable transportation.
