Travel Physical Therapist (PT) – Home Health Medford, New York We are seeking a Travel Physical Therapist for a Home Health contract in Medford, New York . Position Highlights Home Health Setting Travel Contract Full-Time Schedule Adult & Geriatric Patient Population One-on-One Patient Care Independent Field-Based Role Medford, New York Responsibilities Perform comprehensive physical therapy evaluations and reassessments. Develop individualized treatment plans based on physician orders and patient goals. Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients.
Provide gait training, balance training, transfer training, therapeutic exercise, fall prevention, and functional mobility interventions. Educate patients and caregivers on home exercise programs, safety strategies, and proper equipment use. Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
