Travel Physical Therapist (PT) – Home Health Nashville, Tennessee We are seeking a Travel Physical Therapist for a Home Health contract in Nashville, Tennessee . This opportunity is a great fit for an experienced PT who enjoys independent field-based care, one-on-one patient treatment, and helping patients improve mobility, strength, safety, and functional independence in their homes. The therapist will evaluate patients, develop individualized plans of care, provide skilled treatment, and coordinate closely with physicians and the interdisciplinary home health team.
Position Highlights Home Health Setting Travel Physical Therapist Contract Adult & Geriatric Patient Population Independent Field-Based Role One-on-One Patient Care Nashville, Tennessee Start Date: 10/19/26 End Date: 12/14/26 Responsibilities Perform comprehensive physical therapy evaluations and reassessments in the home setting. Develop individualized plans of care based on patient diagnoses and functional goals. Provide therapeutic exercise, gait training, balance training, transfer training, and mobility interventions.
Assess patients for appropriate assistive devices and durable medical equipment. Educate patients and caregivers on safety, home exercise programs, and fall prevention. Monitor patient progress and adjust treatment plans as appropriate.
Coordinate care with physicians, nursing staff, and other members of the interdisciplinary team. Complete accurate and timely clinical documentation. Requirements Tennessee PT license required.
Current CPR/BLS certification required. 1–2 years of PT experience required. Home Health experience preferred. Valid driver’s license required.
Reliable transportation and auto insurance required. Graduate of an accredited Physical Therapy program required. Why Nashville?
