Home Health Physical Therapist

Pride Health is actively hiring a Physical Therapist (PT) to join our client's Home Health Team. Location: Parsippany, NJ, 07054 Coverage Areas: Morris County, NJ

$61.00/hour W2

Monday–Friday, 8:00 AM–4:30 PM Hours: 40 hours/week | Productivity: 30 points/week Weekend: 1 weekend/month Mileage: Reimbursed between patient visits

Provide skilled physical therapy services to patients in their homes. Evaluate patients and develop individualized Plans of Care. Provide therapeutic exercises, gait/balance training, mobility, and functional training.

Educate patients and caregivers on exercises, safety, and home programs. Monitor patient progress and coordinate care with the healthcare team. Complete timely clinical documentation.

Active NJ Physical Therapist (PT) licenseHome Health/Home Care experience requiredCPR certificationValid driver's license, auto insurance, and reliable vehicleAbility to travel throughout Morris CountyStrong communication and clinical skills
