Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risksDevelop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independenceDeliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety educationEducate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniquesComplete accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systemsCoordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aidesCommunicate changes in patient condition, safety concerns, or barriers to progress promptly to the care teamParticipate in case conferences, care coordination activities, and team collaborationEnsure compliance with CMS regulations, infection control standards, and agency policiesManage a mobile caseload efficiently while meeting productivity and documentation expectationsAdapt quickly to new environments, workflows, and patient populations as a travel clinician Required Qualification/CertificationGraduate of an accredited Physical Therapy programActive Physical Therapist license in the state of assignmentHome Health experience requiredWillingness to travel within the assigned coverage areaCurrent BLS certificationValid driver’s license.
