Travel Physical Therapist (PT) – Home Health The Villages, Florida We are seeking a Travel Physical Therapist for a Home Health contract in The Villages, Florida . Position Highlights Home Health Setting Physical Therapist Contract Full-Time Schedule One-on-One Patient Care Independent Field-Based Role The Villages, Florida Start Date: 9/2/26 End Date: 11/30/26 Responsibilities Perform physical therapy evaluations and reassessments in the home setting. Develop and implement individualized patient care plans. Treat patients of varying ages and diagnoses to improve mobility, strength, safety, and independence.
Provide gait training, balance training, therapeutic exercise, transfer training, and functional mobility interventions. Educate patients and caregivers on home exercise programs, safety strategies, and equipment use. Collaborate with physicians, nurses, case managers, and the interdisciplinary care team.
