Preferred Ardor Health Solutions is seeking an experienced Physical Therapist for a home health travel contract with MaineHealth Care at Home in beautiful coastal Maine! This opportunity is ideal for a PT who enjoys working independently, providing personalized one-on-one patient care and helping patients regain mobility and independence in their own homes. Position Overview As a Home Health Physical Therapist, you'll provide skilled therapy services to patients recovering from illness, injury, surgery and other medical conditions.
You'll work with patients in their home environments, developing individualized treatment plans focused on improving functional mobility, safety and quality of life. Responsibilities Conduct comprehensive physical therapy evaluations in patients' homes Develop and implement individualized treatment plans Provide gait training, balance activities and therapeutic exercise Address mobility limitations, weakness and fall risks Assist patients with transfers and functional independence Educate patients and caregivers on safety and home exercise programs Collaborate with nursing, OT, SLP and other home health professionals Maintain accurate clinical documentation Travel between patient homes within the assigned service area Qualifications Active Maine Physical Therapist license BLS/CPR certification Minimum 3 years of PT experience Home health experience preferred Strong independent clinical judgment and communication skills Ability to travel between patient visits Schedule & Assignment Details Day shift, approximately 8:00 AM – 5:00 PM 36 guaranteed hours per week Some weekend coverage required, with compensatory days off No on-call responsibilities Service area and reporting location to be confirmed About Coastal Maine Maine offers an incredible combination of charming coastal communities, beautiful beaches, historic harbors and outdoor recreation.
