Travel Home Health Registered Nurse (RN) Assignment Details Location: Barre, VT Contract Length: 20 Weeks
Monday–Friday, Days Guaranteed Hours: 36 hours per week Potential Hours: Up to 40 hours per week Responsibilities Provide skilled nursing care to patients in their homes according to physician orders and individualized care plans. Perform comprehensive patient assessments and develop, implement, and evaluate care plans. Serve as the primary Case Manager when assigned, coordinating care with physicians, therapists, social workers, and other healthcare professionals.
Educate patients and caregivers on disease management, medications, and home care strategies. Maintain accurate and timely clinical documentation. Qualifications Active Vermont RN license (or eligibility to obtain prior to start) Current BLS certification Recent Home Health nursing experience required
