Monday–Friday, 8:00 AM–4:30 PM; on-call 2–3 nights/week and 1 weekend every 3–4 weeksLocation: Warren, Edmonson, Hart & Butler Counties, KY Float / Coverage Area: Four-county service areaJob Type: Local contract Contract Length: 13 Weeks Mileage Reimbursement Rate (if applicable): $0.48/ mile*Pay offered is based on experience, expertise, credentialing, and education. Duties: Perform comprehensive OASIS assessments, admissions, recertifications, and other required home health visits.
Provide routine skilled nursing care and monitor patients' conditions throughout the course of treatment. Develop and implement individualized plans of care and communicate changes in patient status to physicians and the interdisciplinary care team. Educate patients and caregivers on medications, disease management, safety, and home care needs.
Complete timely clinical documentation while effectively managing a daily field schedule across the assigned service area.
