Job Details:

• Monday–Friday: 8:00 AM–4:30 PMLocation: Columbia, SC – All of Richland CountyJob Type: Local ContractContract Length: 15-weeksPay Range: $52–$54/hourStart: 9/21/26Pay offered is based on experience, expertise, credentialing, and education.Duties:
• Complete admissions to home health services and perform comprehensive patient assessments.
• Provide case management and coordinate individualized plans of care.
• Provide oversight and guidance to LPNs as part of the patient care team.
• Evaluate wounds and support appropriate wound care management.
• Maintain timely documentation and communicate effectively with patients and the healthcare team.
