We are seeking a compassionate and experienced Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will coordinate care with physicians, patients, families, and the interdisciplinary team while developing and managing individualized plans of care. Key
Conduct initial and comprehensive nursing evaluations and determine patient eligibility and medical necessity for home health services. Develop, implement, and revise individualized Plans of Care (POC) in collaboration with physicians and healthcare professionals. Complete accurate OASIS assessments, including medication reconciliation.
Provide high-quality skilled nursing services in accordance with the plan of care, scope of practice, and infection-control standards. Accurately and timely document patient visits and required clinical information in accordance with payer and agency requirements. Supervise and provide direction to Home Health Aides and LPN/LVNs as permitted by state and federal regulations.
Communicate patient needs, changes in condition, orders, scheduling updates, and other relevant information to physicians and agency staff. Participate in case conferences, performance improvement initiatives, survey readiness, and utilization management activities. Work collaboratively with the interdisciplinary team to improve patient outcomes, safety, and reduce unnecessary hospitalizations.
Assist with orientation and serve as a preceptor for new staff and students when needed. Maintain schedule flexibility to meet patient-care needs, including weekend/on-call rotation and potential holiday coverage. Required
