Floyd Valley Healthcare is seeking an experienced Home Health Registered Nurse (RN) to provide high-quality nursing care to patients in their homes throughout Plymouth County, Iowa. The ideal candidate will have at least 2 years of experience performing OASIS assessments and developing care plans for home health patients . Experience with the Epic electronic health record (EHR) system is preferred.
Key Responsibilities Provide skilled nursing care to patients in their homes. Perform OASIS assessments and develop individualized patient care plans. Assess patient conditions and monitor progress toward established care goals.
Coordinate care with physicians, patients, families, and other healthcare professionals. Maintain accurate and timely clinical documentation. Promote patient safety, independence, and continuity of care.
Participate in the department's on-call rotation, including nights, weekends, and holidays. Required Qualifications Current Registered Nurse (RN) license valid in Iowa. Minimum of 2 years of experience with OASIS assessments and home health care planning .
Strong clinical assessment, communication, and care coordination skills. Ability to work independently while providing care in patients' homes. Preferred Qualification: Previous experience using the Epic electronic health record system.
Work Schedule
Day shift Working Days: Monday–Friday Hours: 32–40 hours per week On-Call Rotation: One in four nights, including weekends and holidays. Important Notes Candidates must have substantial experience with OASIS assessments and care planning for home health patients. Applicants should also be comfortable participating in the required on-call rotation.
