We are seeking a compassionate and skilled Home Health Registered Nurse to join our collaborative clinical team. In this role, you will provide individualized nursing care, manage a dedicated caseload, and help patients recover, manage chronic conditions, and maintain their independence at home. Home health nursing gives you the opportunity to slow down, focus on one patient at a time, and make clinical decisions based on what each patient truly needs—all while having the autonomy to manage your day with the support of an experienced interdisciplinary team.

Your responsibilities will include:Completing comprehensive initial and ongoing nursing assessments in patients’ homesEvaluating each patient’s physical, psychosocial, environmental, and safety needsDeveloping, implementing, coordinating, and updating individualized Plans of CareProviding skilled nursing services in accordance with physician orders, agency standards, and applicable regulationsMonitoring patients’ conditions, progress, medications, symptoms, and response to treatmentPerforming follow-up visits based on each patient’s clinical needs and Plan of CareEducating patients and caregivers on diagnoses, medications, treatments, disease management, self-care techniques, and signs that require clinical attentionCoordinating care with physicians, therapists, social workers, home health aides, facilities, and other members of the interdisciplinary teamCommunicating significant findings, changes in condition, safety concerns, or environmental risks promptly to the appropriate clinical personnelRevising the Plan of Care when patient needs change and obtaining updated physician orders as requiredHelping patients and caregivers access appropriate clinical, family, and community resourcesTaking proactive steps to reduce avoidable hospitalizations and emergency department visitsMaintaining timely, accurate, and complete clinical documentationProtecting patient and employee privacy and confidentiality in accordance with HIPAA requirementsParticipating in quality-improvement, safety, and patient-outcome initiativesPerforming additional duties and special projects as assignedWhat You’ll NeedGraduate of an accredited nursing programCurrent, unrestricted Registered Nurse license in the applicable stateAt least one year of nursing experience in a clinical care settingAssociate Degree in Nursing required; Bachelor of Science in Nursing preferredPrevious home health or hospice experience preferredHomecare Homebase experience preferredStrong clinical assessment, communication, organization, and care-coordination skillsAbility to manage an assigned caseload and work independently in the fieldAbility to collaborate effectively with patients, caregivers, physicians, and an interdisciplinary clinical teamDependable vehicle, valid driver’s license, and current automobile insurance in accordance with state requirementsWillingness and ability to travel within the assigned branch coverage areaSit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
