We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care throughout the Red Bud service area. The RN will manage a caseload of home health patients, perform OASIS assessments, provide follow-up visits, and deliver specialized nursing services including wound care and IV therapy. Key
Complete OASIS assessments, admissions, recertifications, follow-up visits, and discharge planning activities. Perform comprehensive patient assessments and develop individualized plans of care in collaboration with physicians and interdisciplinary teams. Provide skilled nursing services including wound care, IV therapy, medication management, disease management, and patient education.
Monitor patient progress, identify changes in condition, and communicate clinical findings to appropriate healthcare providers. Maintain accurate and timely documentation utilizing Homecare Homebase (HCHB). Participate in rotating weekday and weekend on-call coverage as assigned.
Required
Active Registered Nurse (RN) License Recent Home Health Experience Required Homecare Homebase (HCHB) Experience Required OASIS Experience Required Strong wound care and IV therapy skills Current Driver's License, Reliable Transportation, and Auto Insurance
