Key Responsibilities Complete OASIS documentation accurately and timely. Perform comprehensive patient and caregiver assessments. Develop and manage individualized plans of care.
Serve as the case manager for assigned patients. Provide nursing interventions and high-tech care, including IV care, wound VAC, and complex wound care. Collaborate with LPNs and other healthcare professionals.
Monitor and evaluate patient progress and response to care. Provide patient and caregiver education. Coordinate interdisciplinary services such as physical therapy and social work.
Coordinate patient discharges and transfers. Communicate with physicians, providers, vendors, and other care team members. Conduct and participate in case conferences and peer-review audits.
Advocate for patient and family needs. Maintain accurate clinical documentation and follow agency, state, and federal regulations.
