Seeking an experienced Home Health RN to provide skilled nursing care to patients in their homes, coordinate Plans of Care, educate patients/families, and collaborate with the interdisciplinary care team.

Current RN license – Required2+ years Home Health experience – RequiredHCHB (Homecare Homebase) experience – RequiredOASIS experience – Required1–2 years clinical RN experienceCurrent CPR certificationValid driver's license, reliable transportation & insuranceADN/Nursing Diploma or BSN (Preferred) from an accredited program – RequiredKey

Patient assessment, OASIS, wound care, IV/venipuncture, medication administration, care coordination, patient education, and clinical documentation.
