Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team. Location: St. Louis, MO, 63141Pay: $52.00/hr W-2Contract: 14 Weeks | Potential ExtensionSchedule: Monday–Friday | 9:00 AM–5:00 PMHours: 40 hrs/week | 8-hour shiftsJob

We are seeking an experienced Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will assess patient needs, develop and manage Plans of Care, coordinate with interdisciplinary teams, educate patients and families, and ensure quality and continuity of care. Key

Perform comprehensive assessments of home health/hospice patients and develop, implement, and evaluate Plans of Care. Provide skilled nursing care, including wound care, IV/venipuncture, catheter/drain care, phlebotomy, inhalation therapy, and pain management. Maintain accurate and timely clinical documentation and patient records.

Educate patients and families on medications, diet, safety, disease management, and self-care. Coordinate care with physicians, patients/families, caregivers, and interdisciplinary team members. Supervise and provide clinical direction to HHA/STNA/LVN staff as appropriate.

Ensure compliance with payer, infection control, and regulatory requirements. Participate in interdisciplinary team meetings and provide on-call services as assigned. Travel locally to patient homes and work independently while maintaining effective communication with the care team.

Required

Current Registered Nurse (RN) license in the state of practice.2+ years of Home Health experience required.1–2 years of clinical nursing experience required. Homecare Homebase (HCHB) experience – Required. OASIS experience – Required.

Current CPR certification – Required. Valid driver's license, reliable transportation, and auto insurance. Strong clinical assessment, communication, organization, and documentation skills.

Ability to work independently and travel locally.
