We are seeking an experienced Home Health RN to provide skilled nursing care to patients in their homes and coordinate care with physicians, families, and interdisciplinary teams. Registered Nurse (RN) – Home HealthLocation - Tucson, AZ Contract length - 13 weeks ResponsibilitiesPerform comprehensive patient assessments and OASIS assessments. Develop, implement, and update individualized Plans of Care.
Provide skilled nursing, wound care, medication management, IV/venipuncture, catheter/drain care, and pain management. Educate patients and families on medications, treatment plans, safety, and self-care. Supervise HHA/STNA/LVN staff and coordinate patient services.
Maintain accurate clinical documentation and ensure compliance with payer and regulatory requirements. Participate in case conferences and provide on-call support as assigned. Travel locally to patient homes and work independently.
RequirementsCurrent RN license2+ years of Home Health experienceOASIS experience – RequiredHomecare Homebase (HCHB) experience – RequiredCurrent CPR/BLS certificationValid driver's license, reliable transportation, and insuranceADN/Diploma in Nursing required; BSN preferredCHPN certification preferred
