We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care and case management services throughout the Norfolk and Virginia Beach service areas. The RN will perform admissions, routine home health visits, recertifications, and discharge planning while delivering exceptional patient-centered care in the home setting. Key Responsibilities Complete Admissions, Recertifications, Discharges, and Routine Skilled Nursing Visits for home health patients.
Perform OASIS assessments , develop individualized plans of care, and maintain accurate clinical documentation. Provide skilled nursing services including wound care, wound VAC management, Foley catheter care, laboratory specimen collection, medication management, and disease management education . Educate patients and caregivers regarding diagnoses, medications, treatment plans, home safety, and self-care strategies.
Coordinate care with physicians, therapists, caregivers, and interdisciplinary healthcare teams to ensure continuity of care and optimal patient outcomes. Participate in weekday and weekend on-call rotations as assigned. Required Qualifications Active Registered Nurse (RN) License Recent Home Health Experience Required OASIS Experience Required Strong assessment and clinical documentation skills Current Driver's License, Reliable Transportation, and Auto Insurance
