Job Details:

• Monday-Friday | 8:00 AM-4:30 PM | 36 Guaranteed Hours/Week | On-Call/Weekend: NoLocation: Wilson, NCJob Type: ContractPay Range: $52-$54*Pay offered is based on experience, expertise, credentialing, and education. Duties:
• Manage and coordinate patient care as an RN Case Manager.
• Assess patient needs and develop individualized plans of care.
• Coordinate services and communicate with physicians and interdisciplinary healthcare teams.
• Complete accurate and timely patient documentation and follow-up.
