Details Home health setting providing personalized nursing care to hospice patients in their homes Covers the West side coverage area including Petoskey, East Jordan, Boyne City, Harbor Springs, and Mancelona, with potential coverage up to Mackinaw City along I-75 West Day shift schedule, 8:00 AM-5:00 PM Weekend rotation required No on-call requirements Uses Home Care Home Base EMR system Point of Care documentation practice Requirements Active MI RN license required at time of submission Compact license not accepted BLS certification (AHA) Minimum 2 years of hospice experience required Home Care Home Base experience required Oasis experience preferred Travel nursing experience preferred Additional Information Provides skilled nursing care and health counseling to patients in their homes to support treatment, rehabilitation, and symptom management Conducts initial visits to assess patient physical and emotional status through observation, interview, and record review Develops nursing care plans in accordance with physician treatment plans and provides patient teaching Works under the supervision of the Clinical Services Supervisor Matching scrubs required for dress code Candidates residing within a 35-mile radius of the facility are considered local travelers First-time travelers are welcome to apply
