Days | 40-hour salaried position | 8:00 AM - 5:00 PM Compensation $85,000 - $97,000 annually Join an established hospice organization with approximately 20 years of experience and a strong, long-tenured team. Hospice/Home Health experience is preferred, but experienced LTC RNs will also be considered. This role focuses on compassionate, patient-centered end-of-life care using a whole-person mind, body, and spirit approach.
Complete comprehensive patient and family assessments Develop and revise individualized plans of care Provide skilled nursing care and symptom management Coordinate care with physicians, facilities, families, and the interdisciplinary team Maintain timely and accurate EMR documentation Order medications, DME, and supplies Supervise ancillary staff and coordinate visits Participate in on-call rotation and IDG meetings
