Discharge planning and care coordination Psychosocial assessments Hospice and post-acute care planning Patient/family counseling and support Community resource referrals Collaboration with physicians, nursing, and interdisciplinary teams Requirements Active CA LCSW license is needed of all applicants! 2 yrs experience working as an LCSW in an acute care hospital setting EPIC EMR experience is preferred About the Facility Join a highly respected 515-bed Magnet-designated acute care hospital serving the greater Los Angeles area.
