Long Term Care (LPN / LVN)

8-Hour Days (7 AM – 3 PM) or Evenings (3 PM – 11 PM)OT Rate: $67.50 / hrResponsibilities:Administer scheduled medications and treatments per physician orders. Monitor vitals, assess residents, and report status changes to RNs. Complete accurate daily EMR documentation and point-of-care charting.

Assist residents with ADLs and collaborate with CNAs and multidisciplinary staff.

Active NY LPN License (unencumbered). Current AHA or ARC BLS certification.1+ year recent LTC/SNF experience preferred. Available for Day or Evening shifts, including rotating weekends.
