Long Term Care (RN)

Provide nursing care to long-term care and geriatric patients. Perform assessments and monitor patients' conditions. Administer medications and treatments as ordered.

Develop and update patient care plans. Monitor vital signs and report changes in patient condition. Provide wound care and other nursing interventions as needed.

Collaborate with physicians and other healthcare professionals. Educate patients and families regarding care and treatment. Maintain accurate patient records and documentation.

Follow facility policies, safety standards, and infection-control procedures.

Active RN license in the state of practice. BLS/CPR certification. Previous LTC/SNF experience preferred.
