The LPN will work closely with RNs, physicians, CNAs, therapists, and other members of the healthcare team to support patient recovery and ensure safe, effective care. ResponsibilitiesProvide direct nursing care to patients under the direction of an RN and/or physician. Monitor and document patient conditions, vital signs, and changes in health status.
Administer medications and treatments as ordered. Assist with patient assessments and communicate changes to the appropriate healthcare team. Provide wound care and other treatments within scope of practice.
Assist patients with activities of daily living and mobility as needed. Maintain accurate and timely documentation in the electronic medical record. Follow infection prevention, patient safety, and facility policies.
Collaborate with the interdisciplinary healthcare team. Provide education and support to patients and families.
