  • AMDIJP00011202 LPN Home Health Description: Provides coordinates and directs the provision of home nursing care according to physicians orders based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager Directly Supervises and Evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. DEGREE OF TRAVEL Home visits. Periodic office meetings. Must have reliable transportation current drivers license and agency-required liability insurance.' Average # of patients/day: 5
  • 8 Points per day/week: How are points broken down 30 points per week. Each type of visit and activity (documentation travel etc.) hold a different point value. Contractor will be assigned between 5

4x10s Friday-Monday. Most will be standard 5x8s M-F. Hospital/Agency Highlights Dress Code: Scrubs Charting: All professions must be proficient with HomeCare HomeBase (HCHB) and used it within the last 6 months.
