- Graduate from an accredited school of Practical Nursing in the State of Illinois and currently licensed as a Practical Nurse in the State of Illinois.
- BLS certification
- Successful completion of coursework for medication administration (Pharmacology) as a Practical Nurse in accordance with the Illinois Department of Financial and Professional Regulation (IDFPR) regarding Standards of Pharmacology/Administration for Medication Course for Practical Nurses (Must provide documentation if requested)
- Candidates must be available for an in-person interview and successfully pass a Department of Corrections (DOC) background check before being scheduled for an interview. Candidates must provide a copy of a valid State ID or Driver’s License, front and back, and complete a PREA form to complete the screening process. Interview consideration is contingent upon successfully passing the DOC background check.

No RTO on weekends. Will clinician float within scope to meet facility needs? Cook is a MANDATORY FLOAT FACILITY; all resources are required to float at the facility's discretion.
