Day Shift | 8:00 AM–4:30 PM | 5×8-hour shifts Weekly Hours: 40 hours Contract Length: 13 weeks

Master's degree in Social Work (MSW) required. Minimum 2 years of clinical, emergency department (ED), or medical social work experience. Must be working toward Licensed Clinical Social Worker (LCSW) licensure, at minimum.

Active driver's license required. Active BLS certification required. Epic EMR experience.

Must be able to lift 50 lbs intermittently. Must have a vehicle suitable for driving in various conditions, including gravel roads, snow, rain, and ice. Must be able to work with patients of all ages, with the majority over 65 years old.

Conduct biopsychosocial assessments for hospice patients. Provide social work services to patients and families across all age groups. Support patients and families with psychosocial needs associated with hospice care.

Document patient assessments and care in the Epic EMR. Travel to patient locations under varying road and weather conditions.
