Providence Mission Hospital is seeking an experienced Medical Social Worker (MSW/LCSW) to join its acute care inpatient Case Management team. The social worker will be responsible for patient assessments, discharge planning, care coordination, and arranging post-acute services to ensure safe and timely patient transitions. Key Responsibilities Conduct initial biopsychosocial assessments within 24 hours of admission.
Develop and coordinate individualized discharge plans and contingency plans. Coordinate post-acute care services, including referrals to rehabilitation and long-term care facilities. Arrange durable medical equipment (DME), home care services, and other discharge-related needs.
Collaborate with physicians, nursing staff, insurance case managers, and utilization management teams. Assess patient and caregiver needs, establish care goals, and evaluate responses to treatment. Provide patient and caregiver education regarding discharge plans and available community resources.
Identify and address barriers to discharge, including insurance coverage, housing, transportation, and financial assistance. Maintain accurate documentation in Epic, including assessments, care plans, referrals, and patient progress. Provide crisis intervention and connect patients with appropriate social and community support services.
Required Qualifications Master of Social Work (MSW) degree. Minimum of 1 full year of acute care inpatient hospital experience ; 2 years of experience preferred/required per the job profile. Recent hospital experience within the last 3 months.
Previous travel experience required — first-time travelers will not be accepted. Experience in medical social work, hospital case management, and discharge planning. Epic experience is mandatory.
BLS certification. LCSW certification is preferred. Strong knowledge of care coordination, post-acute placement, and community resources.
Important Candidate Requirements Must have acute care inpatient hospital experience. Candidates with specialized Behavioral Health or Emergency Department social work experience only will not be accepted. The role focuses on medical social work, inpatient case management, and discharge planning , not behavioral health or ED social work.
Candidates must have prior travel experience. Floating to the Laguna facility may be required. California state license is not required according to the job details.
Requested time off is limited to a maximum of 7 days. Work Environment The Medical Social Worker will work within an acute care hospital's interdisciplinary Case Management department alongside registered nurses, social workers, and clinical coordinators. The position involves managing an adult inpatient caseload, coordinating post-discharge needs, and facilitating safe, timely transitions to the appropriate level of care.
