Home Health RN Job Details Location: Uniontown, Ohio, serving patients throughout Summit, Portage, and Medina counties Weekly Estimated
$2,120–$2,230 Start Date: October 12, 2026 Contract End Date: January 8, 2027 Contract Duration: Approximately 13 weeks
8-hour shifts, approximately 8:00 a.m.–4:30 p.m.; 40 hours per week with 36 hours guaranteed On-Call and Weekend Requirement: One on-call week and one weekend during the assignment Travel: Mileage reimbursement between patient visits, from the first patient to the last patient Patient Care Setting: Home health Dress Code: Navy or black scrubs Home Health Registered Nurse Job Requirements Current Registered Nurse (RN) license in the state of Ohio At least 1 year of professional Registered Nurse experience Associate degree in Nursing or equivalent educational preparation Required professional nursing certifications and credentials in good standing Strong patient assessment, clinical judgment, documentation, and care coordination skills Valid driver’s license and reliable transportation for community-based home health visits Previous home health nursing or case management experience preferred Experience with Homecare Homebase (HCHB) is a plus Home Health RN Case Manager Responsibilities Serve as the case manager for an assigned home health patient caseload Perform comprehensive nursing assessments and develop individualized plans of care Provide skilled nursing interventions and monitor patient progress in the home setting Coordinate services with physicians, therapists, aides, caregivers, and other members of the interdisciplinary care team Educate patients and families about medications, disease management, treatment plans, and home safety Identify changes in patient condition and communicate clinical concerns promptly Complete accurate and timely clinical documentation in accordance with home health standards Support continuity of care through follow-up visits, reassessments, and interdisciplinary collaboration Provide on-call and weekend coverage as scheduled Apply now for this Home Health Registered Nurse (RN) Case Manager contract opportunity in Uniontown, Ohio, and help patients receive exceptional care in the comfort of home.
