- 45/hour Duties and Responsibilities- Plan, execute, coordinate, assess, and support continuity of care in accordance with established standards and best practices.- Evaluates health status by discussing the patient’s current condition and reviewing medical history.- Identifies patient and family needs and delivers education and instructional resources as appropriate.- Offers psychosocial assistance to patients and their families.- Supports clinical workflows and facilitates patient care activities.- Handles patient medication refill requests received through the pharmacy phone line and documents details in the patient record.- Coordinates and arranges consultations, pre-procedure testing, diagnostic examinations, referrals, and follow-up appointments for patients.- Reviews and prioritizes patient phone calls as needed and records relevant information in the patient's medical chart.- When required, serves as a liaison between the patient/family and interdisciplinary care team while coordinating patient-related services. Requirements and QualificationsActive RN LicenseOncology experience preferredICU or ER background3+ yrs nursing experienceBLS Certification 26
