To Be Confirmed Contract: Travel Assignment Ardor Health Solutions is seeking a Physical Therapist for an outpatient geriatric rehabilitation contract at The Heritage Tradition in Sun City, Arizona! This is a fantastic opportunity for a PT who enjoys working with older adults in a personalized, one-on-one treatment environment. Unlike traditional high-volume outpatient clinics, this position focuses on providing individualized therapy services to residents in an assisted living community.
Position Overview You'll provide outpatient physical therapy services to geriatric patients within their senior living community, helping residents maintain mobility, improve balance, prevent falls and preserve their independence. The one-on-one treatment model allows you to build meaningful relationships with patients while focusing on their individual rehabilitation goals. Responsibilities Conduct comprehensive physical therapy evaluations Develop individualized treatment plans for older adults Provide one-on-one outpatient therapy Address gait, balance, strength and functional mobility Implement fall prevention and safety programs Provide therapeutic exercise and functional rehabilitation Assist residents with maintaining independence in daily activities Educate patients and caregivers Collaborate with community staff and other healthcare professionals Maintain accurate clinical documentation Qualifications Active Arizona Physical Therapist license Graduate of an accredited physical therapy program Experience with geriatric rehabilitation preferred Strong clinical assessment and communication skills Passion for providing individualized patient care About Sun City, Arizona Located in the northwest Phoenix metropolitan area, Sun City offers beautiful desert scenery, warm weather and convenient access to everything the Valley of the Sun has to offer.
