Required qualifications Active Louisiana pharmacist license Doctor of Pharmacy (PharmD) or BS Pharmacy degree BLS through AHA required Hospital pharmacy experience is preferred, not mandatory according to the posting ACLS through AHA is preferred Responsibilities Prepare, verify, and dispense medication orders. Review medication therapy for safety, interactions, allergies, contraindications, and appropriate dosing. Compound sterile and nonsterile medications, including chemotherapy and parenteral nutrition.
Support renal dosing, pharmacokinetics, anticoagulation, and IV-to-oral conversions. Participate in antibiotic and opioid stewardship, medication-use reviews, and interdisciplinary rounds. Provide drug information and education to patients and healthcare staff.
