🏡 Travel Physical Therapist (PT) – Home Health 📍 Rhode Island Ready for your next Home Health adventure? Ardor Health Solutions is seeking an experienced Travel Physical Therapist for an exciting Home Health assignment serving patients throughout Rhode Island. If you enjoy the flexibility and autonomy of Home Health while making a meaningful impact in patients' lives, this is an outstanding opportunity!

Perform comprehensive physical therapy evaluations in patients' homes. Develop individualized treatment plans based on physician orders and patient goals. Treat patients to improve mobility, strength, balance, and functional independence.

Complete OASIS assessments and maintain accurate, timely documentation in HCHB. Educate patients and caregivers on home exercise programs and safety strategies. Collaborate with physicians and the interdisciplinary care team to ensure optimal patient outcomes.

Supervise Physical Therapist Assistants in accordance with state practice guidelines. Assignment Highlights ✅ 40 hours guaranteed each week ✅ Monday–Friday | 8:00 AM – 5:00 PM ✅ Work only 2 weekend days per month ✅ No on-call responsibilities ✅ Mileage reimbursement provided ✅ Coverage area includes Providence, Bristol, Kent, and Washington Counties ✅ Competitive weekly pay package. ✅ Medical, Dental & Vision Insurance. ✅ 401(k) with company match. ✅ Licensure reimbursement & CEU assistance. ✅ Dedicated recruiter support throughout your assignment. ✅ Access to nationwide travel therapy opportunities.

Requirements Active Rhode Island Physical Therapist license or Compact PT License. Home Health experience required. HCHB experience required.
