Provide skilled physical therapy services to patients in their homes. Complete comprehensive evaluations and develop individualized plans of care. Help patients improve strength, balance, mobility, safety, and functional independence.

Provide gait, transfer, and therapeutic exercise training within the patient's home environment. Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility. Collaborate with physicians, nurses, and other members of the Home Health care team.

Complete accurate and timely clinical documentation. Requirements Active Maine Physical Therapist license or eligibility to obtain. Previous Home Health experience preferred.

Current BLS certification. Strong communication, time-management, and independent clinical decision-making skills. Ability to travel between patient homes as required by the assignment.
