🏡 Travel Physical Therapist (PT) – Home Health 📍 Gassaway, WV Ready for your next Home Health opportunity? Ardor Health Solutions is seeking a Travel Physical Therapist for a rewarding Home Health assignment based out of Gassaway, West Virginia! This is a great opportunity for a PT who enjoys one-on-one patient care, independence, and a consistent weekday schedule.
Perform comprehensive physical therapy evaluations in patients' homes. Develop individualized treatment plans based on physician orders and patient goals. Help patients improve mobility, strength, balance, safety, and functional independence.
Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility. Collaborate with physicians and the interdisciplinary healthcare team. Complete accurate and timely clinical documentation.

Requirements West Virginia PT license can be pending — you do not need to have it in hand to get started! Home Health experience preferred. OASIS experience preferred.
Current BLS certification. Valid driver's license and reliable transportation. Strong communication, organization, and independent clinical skills.
