Physical Therapist - Home Health

Travel PT Home Health jobs in Blue Bell, PA covering Montgomery County offer 13-week contracts with 8-hour days, Monday through Friday, and 40 hours expected weekly. You must have a Pennsylvania license, home health experience, and Start of Care experience. You will visit 5 to 6 clients daily and document using Homecare Home Base.

Mileage is reimbursed at $0.72 per mile. Your responsibilities include evaluating patients, developing individualized care plans, implementing skilled interventions, monitoring progress, and educating patients and families on home exercise programs and safety. You will collaborate with a multidisciplinary team and coordinate care with nursing, occupational therapy, and social work.
